Drivers receive a checklist of required items prior to their appointment to ensure a smooth visit.


Exams are completed at your location with minimal disruption to your operations. Most physicals take approximately 20–30 minutes per driver.

Efficient On-Site DOT Exams


Drivers who arrive prepared and meet certification requirements receive their Medical Examiner’s Certificate promptly once exam documentation and payment are finalized. Required FMCSA/NRCME reporting is submitted electronically, and documentation is completed promptly to support state DMV compliance.

Before Your Appointment


To ensure a smooth and efficient visit, there are two important steps to complete before your appointment:


Step 1: Required Driver Intake Form

All drivers must complete a secure online intake form prior to the appointment. This includes medical history questions and a full list of current medications with dosages.

Complete the intake form using the button or QR code below. The form can be completed on a smartphone, tablet, or computer with internet access.

The intake typically takes 5–10 minutes to complete. Submitting it in advance helps ensure a streamlined visit and same-day certification for qualified drivers.

Please Note: the intake form may auto-generate a letter for you to take to your primary care provider, this may not be necessary but beware this will be an area of discussion during your physical and any documentation you might already have from your doctor could be sufficient and please bring it with you.

Step 2: Bring Required Items

Valid US driver’s license (must be current and up to date with the DMV)

Glasses or contact lenses (if required for driving)

Hearing Aids (if applicable)

Medical documentation for ongoing conditions (if applicable — see checklist for details)

Obstructive sleep apnea reports (if applicable — see checklist for details)

All current medication and supplement information (only needed if completing the intake form on-site)

Arriving prepared helps keep your visit on schedule and allows most drivers to complete their exam and receive certification the same day.
