Efficient On-Site DOT Exams
Drivers who arrive prepared and meet certification requirements receive their Medical Examiner’s Certificate promptly once exam documentation and payment are finalized. Required FMCSA/NRCME reporting is submitted electronically, and documentation is completed promptly to support state DMV compliance.

Before Your Appointment
To ensure a smooth and efficient visit, there are two important steps to complete before your appointment:
Step 1: Required Driver Intake Form
All drivers must complete a secure online intake form prior to the appointment. This includes medical history questions and a full list of current medications with dosages.
Complete the intake form using the button or QR code below. The form can be completed on a smartphone, tablet, or computer with internet access.
The intake typically takes 5–10 minutes to complete. Submitting it in advance helps ensure a streamlined visit and same-day certification for qualified drivers.

Step 2: Bring Required Items
Valid US driver’s license (must be current and up to date with the DMV)
Glasses or contact lenses (if required for driving)
Hearing Aids (if applicable)
Medical documentation for ongoing conditions (if applicable — see checklist for details)
Obstructive sleep apnea reports (if applicable — see checklist for details)
All current medication and supplement information (only needed if completing the intake form on-site)
Arriving prepared helps keep your visit on schedule and allows most drivers to complete their exam and receive certification the same day.

Trail Ridge Medical provides mobile, on-site DOT physical exams at your business location. A private indoor space and restroom access are required to ensure proper vision and hearing testing, confidential medical discussion, and exam components.
For professionalism and privacy, exams are not conducted in parking lots, truck stops, or roadside locations.

Yes. Travel within 60 miles (one way) of Idaho Falls — including Driggs, St. Anthony, Dubois, and Pocatello — is included at no additional cost.
For locations beyond 60 miles (one way), a flat travel fee applies:
• 61–90 miles → $75 travel fee
• 91–120 miles → $150 travel feeFor locations more than 120 miles away (one way), minimum booking requirements and additional travel costs may apply.
For locations outside Eastern Idaho, travel may be available with advance notice and minimum booking requirements. Please contact me directly to discuss scheduling and logistics.

Payment is collected on the day of service. Trail Ridge Medical accepts all major credit and debit cards, including Apple Pay and Google Pay.
Cash payments may be accepted (exact amount preferred).
Checks are not accepted.
For individual drivers, payment is secured prior to completing the exam. DOT Medical Examiner’s Certificates are provided once payment has been received.
For company bookings, a secure payment link can be sent in advance or at the time of service. Certificates will be issued once payment is received and can be provided electronically if needed.

Most DOT physical exams take approximately 20–30 minutes per driver. Time may vary depending on medical history and required documentation.

Medical Examiner Certificates are issued once the exam is completed and all required documentation and payment have been finalized. Certificates can be provided immediately on site or sent electronically when applicable.

Yes. On-site visits can be scheduled for individual drivers or groups. Drivers will be scheduled back-to-back during the same visit time block to minimize downtime and keep operations moving.

Scheduling in advance is recommended to ensure availability, especially for group visits. Efforts are made to accommodate urgent needs when possible.

Trail Ridge Medical currently provides DOT physical exams only.
Urine drug screens may be offered soon.
Additional occupational health services may be added in the future as the practice expands.

If additional medical documentation is required, clear instructions will be provided. Once documentation is received and reviewed, certification can be finalized as appropriate.
When possible, efforts are made to help drivers understand next steps to support compliance.

Drivers are strongly encouraged to complete the secure online intake form prior to arrival to help keep the visit efficient and on schedule.
The form is completed electronically and can be accessed through a secure, unique link or QR code provided at the time of scheduling or displayed on the Trail Ridge Medical website. It may be completed on any smartphone, tablet, or computer with internet access.
If circumstances prevent completion in advance, the form may be completed on-site using a provided device and internet connection. Please note this will add time prior to the exam.
